IMPORTANT UPDATE for MoneyGuard in All States
All States LTC Personal Worksheet: Now required in ALL states at Point of Sale. You are likely familiar with this as a delivery requirement. It is a 3 page worksheet. The client and advisor can still select the box on page 3 indicating they do not wish to complete the questions, but the client and agent still need to sign/date it and return all 3 pages. There are state variations of this form in CA, FL, MA, MD, NC, NJ, NY, PA, TX, VA, VT and there are slight differences on each form so they cannot be used interchangeably. The state specific versions are available at www.lfd.com under the "Forms" section. All other states can use this version. Replacement of Life Insurance Form (LF10087): Now required in ALL states (each state has it's own version). Includes Agent Certification section and is required to be submitted along with other standard paperwork (MG Reserve ticket, Signed Illustration OR Simplified Quote, Temporary Insurance Agreement, 1035/replacement forms, LTC Worksheet, etc) Both changes in Point of Sale paperwork go into affect in all states on Monday, February 21. Cases will not be able to go to LifePlans until these are received.

Paperwork that must be signed now on the FRONT END:
1. MoneyGuard Ticket: advisor signs
2. Simplified Quote or Full SIGNED Illustration
3. LTC Worksheet: client and advisor sign
4. Checklist: client and advisor sign
5. Agent Certification: advisor signs
6. California Disclosure if the client is over age 65: client and advisor sign
